Fullerenes separation with monomeric type C30 stationary phase in high-performance liquid chromatography.

The temperature effect on the separation of fullerenes in LC was examined using monomeric type C30, C18 and C8 alkyl bonded stationary phases. It appears that the C30 phase exhibits superior separation ability for fullerenes. It is observed that the maximum retention temperature of fullerenes on the C30 phase is around 20 degrees C. A strong correlation between the changes in NMR spectra and the retention behavior of the solutes was found. The interpretation of the retention behavior of fullerenes on the alkyl bonded stationary phases, including the behavior in subambient temperature, is discussed using the information obtained by CP-MAS solid-state NMR spectroscopy and LC.

Perception of dioxin and other risks in Japan: replication and extension.

Previous research has shown that the common understanding of risk may be expressed by factor-analytic models that represent how fatal are risks and how voluntarily one assumes such threats. We investigated whether a similar representation would model Japanese participants' risk perception. Moreover, because Japanese newscasts concentrated their coverage exclusively on dioxin during the period of this research, we expected that those who were exposed to such newscasts would more frequently develop greater environmental awareness. We also examined whether this "news-exposed group's" knowledge about dioxin was consistent with the known facts regarding dioxin. Analysis showed that the two factors adequately modeled the Japanese sample's risk representation (N = 473) and that the news-exposed group (n = 188) did exhibit higher environmental awareness. Implications for risk research are discussed.

[Use of ultrasonically activated scalpel for the paced patient--a case report].

A paced patient underwent mitral valve replacement for mitral stenosis using ultrasonically activated scalpel. There were minimum bleeding and no homologous blood transfusion was required. Ultrasonically activated scalpel fid not interfere the pulse generator nor the transesophageal echocardiography. Ultrasonically activated scalpel is useful for the open heart surgery in paced patients.

[A hanger for thoracic surgery].

A new hanger for thoracic operation is introduced. This hanger for draping cloth is made for the purpose 1) of isolating the operative field from the anesthetist, 2) of keeping the broad hygienic area around the operator, 3) of keeping the area to handle the endotracheal tube for the anesthetist, 4) of keeping the area on the cranial side of the patient where the second assistant is able to stand, 5) of keeping the area for the watcher to stand.

[A case of hypertrophic obstructive cardiomyopathy using intraoperative trans-esophageal echocardiography].

A 45-year-old man underwent trans-aortic myectomy for hypertrophic obstructive cardiomyopathy (HOCM). Although two mounds of hypertrophic septal muscle were resected, Brockenbrough phenomena proved unsatisfactory myectomy. Intraoperative trans-esophageal echocardiogram (TEE) revealed a residual mound at the apical septum. After the additional myectomy, Brockenbrough phenomena was disappeared. Intraoperative TEE is an useful supplement in the operation of transaortic myectomy for HOCM.

Fast protein separation by reversed-phase high-performance liquid chromatography on octadecylsilyl-bonded nonporous silica gel. II. Improvement in recovery of hydrophobic proteins.

Recovery of hydrophobic proteins from an RP-HPLC column was improved using a fast-separation RP-HPLC system operated at room temperature. Hydrophobic proteins such as ovalbumin could be adequately eluted from a nonporous octadecylsilyl (C18) spherical silica gel with a particle diameter of 20 microns using steep gradient elution with a 0.1% aqueous trifluoroacetic acid-acetonitrile system at a constant flow rate of 4 ml/min. Recoveries improved under fast separation since the protein sample suffered only a slight amount of irreversible denaturation on the hydrophobic surface of the stationary phase. The fast-separation system was also applied to the separation of larger proteins such as apo-ferritin (443 kDa) and thyroglobulin (669 kDa) as well as egg white proteins.

[Right ventricular thrombosis due to familial heparin cofactor II deficiency].

This case report was with regard to familial heparin cofactor II (HC II) deficiency. The patient was a 14-years-old female, having complaints of chest pain, fever and swelling of right lower extremity. Echocardiography and DSA showed a pediculated mass at the right ventricular outflow tract which highly suggested a ventricular myxoma. At surgery, a large pediculated thrombus was removed from the RV conus septum aun pulmonary valve. The mass was identified as thrombus by histological examination. Two months later, the RV thrombi recurred with additional pulmonary embolus to the left lung. HC II was discovered by hemo-coagulation tests. Her HC II antigen value was 48%, and 32% on repeat study. Her brother's HC II antigen value was 53%. At reoperation, small thrombi less than phi 10 mm, were removed from the RV outflow tract and pulmonary valve. Fresh frozen plasma was given and intravenous urokinase therapy was taken. She is doing well with therapy of oral warfarin, aspirin and ticlopidine for anticoagulation.

[A case of isolated congenital diverticulum of the left ventricule].

Congenital diverticulum of the left ventricule is rare congenital cardiac malformation and is classified as muscular and fibrous. Muscular diverticula are usually accompanied with other cardiac and extracardiac malformations. 8 years old boy involved with muscular left ventricular diverticulum with no other congenital anomalies underwent resection of diverticulum. The cardiopulmonary bypass was required because it was not feasible to apply a clamp, to the neck of the diverticulum. Congenital diverticulum of the heart should be resected as soon as possible because of crisis of spontaneous rupture and systemic embolization. However, the assessment of cardiac arrhythmias and hemodynamic changes caused by clamping of the diverticulum during operation is necessary.

[Obstructive jaundice from benign hepatic cysts].

Obstructive jaundice secondary to benign, non-parasitic hepatic cysts is rare, since there are only 19 previously reported cases. We reported a case of polycystic disease of the liver and kidneys in which relief of developing jaundice was obtained by hepatic lobectomy and fenestration procedure for the multiple cysts. Immense occupancy and porta hepatis proximity of the cysts were triggers for developing jaundice. Urgent resection of the major cysts is recommended for extremely progressing jaundice.

[Spasm in aortocoronary vein graft].

A case of spasm of saphenous vein aortocoronary bypass graft in a 70-year-old man is reported. Six months after coronary artery bypass grafting for three vessel disease, routine coronary angiography revealed narrowings of RCA just distal to the site of graft insertion and midportion of vein graft for right coronary artery. PTCA was performed for these stenoses, and then spasm of the vein graft occurred in other 2 points. Isosorbide dinitrate administration and balloon angioplasty for spastic parts were tried. No spastic narrowing of the vein graft was seen in angiography on the next day.

[An operative case of partial anomalous pulmonary venous connection with patent ductus arteriosus and atrial septal defect].

The patient was a 28-year-old female who had underwent the operation of the closure of ASD on 7 years old. She administered due to palpitation. Cardiac catheterization revealed PDA, residual ASD, and PAPVC that blood flow from right superior pulmonary vein returned to the high level of SVC (juxsta-inominate vein). Successful repair was performed by intraluminal direct closure for PDA and placing a long patch for ASD and PAPVC.

[An operative case of partial anomalous pulmonary venous connection with systemic lupus erythematosus].

The patient was 43 years old female who was administrated by shortness of breath. Preoperative data including immunological examinations identified as SLE. Cardia angiogram showed PAPVC of the right pulmonary veins returned to the right atrium without ASD. Pulmonary hypertention was found with cardiac catheterizations. Open lung biopsy identified pulmonary hypertention caused by shunt volume load, but not by SLE. Surgical correction was accomplished by creating ASD and constructing a baffle to direct the blood flow from the right pulmonary veins to the left atrium. Resulting from this procedure was made pulmonary arterial pressure decreased to normal range.

[Transference of cefmenoxime from serum to lung tissue].

Ten patients who were performed pulmonary resection for the disease of the lung, were administered 2 g of cefmenoxime (CMX) by intravenous injection before their operation. The concentrations of CMX in serum and lung tissue were determined. The CMX concentrations in lung tissue were observed to be higher than the MIC of CMX for Klebsiella pneumoniae, Haemophilus influenzae and Serratia. These results suggested that CMX will be useful agent for the prevention and treatment of pulmonary infection.
